To file a claim for a lost or damaged package, or for orders not received but marked as delivered please read carefully, fill out and submit the form below.
In order to file a claim, you will need to provide the following:
The tracking number (including a screenshot of a tracking page from USPS proving when they accepted the package) For missing USPS shipments, you'll need to wait 20 days from the shipment date for a domestic order, or 40 days from the shipment date for an international order before you can file an insurance claim. That said, if the package has already been scanned as Delivered and you need to file a claim, there is no minimum waiting period.
An invoice or proof of value of all items in your order (i.e. your order confirmation email)
Proof of a payment transaction for your order (i.e. credit card, PayPal, Apple Pay receipt, etc.)
Screenshots of messages or emails you’ve had with USPS if your package has been marked as delivered but you did not receive it.
For items damaged during shipping, you'll need to submit: 1) a photo of the entire damaged item; 2) photos of the external packaging (aka the outside of the envelope); 3) photos of the inside of the envelope with any packaging materials.
Please note that this evidence is necessary in order to begin the claim filing process. Our 3rd-party insurance may or may not request additional evidence based on the circumstances of your claim. Keep in mind not complying with our 3rd-party's additional requests may result in a denial of your claim!
The deadline to file a claim and provide our 3rd-party insurer with everything they need is 120 calendar days from the date your package was accepted by the shipping courier.
When/If the claim is approved by the 3rd-party insurer (usually 2 -3 weeks after you submit the form below), you will receive replacements items (if in stock), a Fablebands Gift Card, or a full refund.
